Orange Blossom Family Health Center
Our health facility, the Orange Blossom Family Health Center, is the delivery site for our many programs and is located just a few blocks west of downtown Orlando. Our programs include primary and preventive medical care, oral health services, behavioral health and substance abuse counseling, an on-site pharmacy, vision services, a residential TB shelter, mobile health services and street outreach - the HOPE Team.
H.O.P.E Team
Our HOPE (Homeless Outreach Partnership Effort) Team works where their clients live…on the streets and in the woods. Through their outreach, they are connecting people who live in the woods of Central Florida with the many health services we can provide at the Orange Blossom Family Health Center. The Team includes two outreach specialists and a Registered Nurse
Primary and Preventive Medical Care
Primary medical care is provided
by a team of physicians, nurse practitioners, physician assistants,
registered nurses, medical assistants, and student interns. Our goal
is to create a "medical home" for our patients in an effort to prevent
the inappropriate use of hospital emergency rooms
Oral Health Services
Dental services at HCCH are provided by
a team of dentists, hygienists, dental assistants, and students. Our
services include emergency dental care, oral screenings, cleanings,
restoration work, and referrals for surgery.
Behavioral Health and Substance Abuse Counseling
Our behavioral health team consists of two licensed mental health counselors and a psychiatric nurse practitioner who provide mental health and substance abuse counseling, individual therapy, and referrals to detoxification and crisis centers. Through individualized treatment plans, our behavioral health team is helping their patients make better life choices
